Barwon Health’s Pharmacy Department have had a standout month celebrating two significant achievements.
Intern pharmacist Kate Ross won the Best Hospital Pharmacy Project award at the Monash University Intern Foundation Program Conference, for her poster “Documentation of Medication Changes During Transitions of Care from the Intensive Care Unit”. Findings support integrating pharmacists in electronic discharge documentation at transitions of care to strengthen communication to multidisciplinary teams and GPs, and improve patient safety and continuity of care.
Workforce Development Pharmacist Sophie Jahnecke received the Dr Erica Sainsbury Regional Health Service Supervisor Award at the Australian Pharmacy Council (APC) Emeritus Lloyd Sansom Distinguished Lecture Series in Canberra. The Dr Erica Sainsbury Awards were established by APC to recognise and reward pharmacists who display the attributes and skills considered invaluable in the training of pharmacy interns during the period of supervised practice.
